As well as the theory of the four humours, Medieval people had religious and supernatural ways of diagnosing illness and disease. A common belief was that disease was a punishment for sinning against God and that therefore you deserved to be ill. Another explanation for disease was astrology, and medically trained physicians would use star charts to diagnose disease and decide how you should be treated.
